Proposal Development

Funding Opportunities

University Research Council (URC) forms

URC Application 
Grant applications are now being accepted for the 2009-10 academic year. Completed applications must be submitted by 4 pm, October 30.

URC Midterm Report
This form is to be submitted by all grant recipients 6 months following the receipt of a URC award in order to record the progress of the recipient's research to date.

URC Budget Revision Form
This form is to be submitted by grant recipients who anticipate any deviation (minor or major) from the proposed budget.

URC Request for Award Extension
Award extensions are approved only on a short-term basis. Those who are requesting extensions for periods longer than one (1) month will be required to provide additional information on a case-by-case basis.

URC Final Report
This form is to be submitted by all grant recipients one (1) year following receipt of a URC award in order to document the successes and/or failures of the proposed research. Please note, PIs who do not submit a final report WILL NOT be eligible for future funding.

Budget

Facilities and Administrative (F&A) Reduction Request Form 
For detailed information on F&A Costs see the Budget Development and Guidance – Facilities & Administrative (F&A) Costs page.  If your proposal meets the guidelines for a F&A reduction, please complete this form and submit to the Office of Sponsored Programs no less than five working days prior to your submission deadline.

Modified Total Direct Costs (MTDC) 
This is the budget format most commonly used for federal grants.  The template will calculate the F&A rate on all costs with the exception of equipment over $5,000, capital improvements, and the first $25,000 on subawards. YELLOW headings on excel spreadsheet.

National Science Foundation (NSF)
This template mirrors the budget categories on an NSF budget. Like the MTDC, it calculates the F&A rate on all costs with the exception of equipment over $5,000, capital improvements, participant support costs, and the first $25,000 on subawards. GREEN headings on excel spreadsheet.

Total Direct Costs (TDC)
This template is to be used if F&A is being charged on all items in the budget. Simply speaking, this template will calculate a flat F&A rate on all expenditures.  If you have a contract with a state agency, you will probably use this template.  BLUE headings in excel spreadsheet.

Cost-share Template
This template is a mirror image of the MTDC and is to be used to calculate any match or cost share that is required by a sponsor.  If you are agreeing to provide cost share, you must complete this template to show how you calculated the costs of the cost share items.  If you are submitting a grant with cost share, please call our office for assistance as these budgets can be complex and complicated.  RED headings in excel spreadsheet.

Research Protections

Internal Review Board (IRB) forms

IRB Application/Request for Review
Request IRB approval of a new research study involving human subjects. Additional materials used in the study (recruiting materials, questionnaires, etc.) should be submitted with the application.

IRB Informed Consent
In almost all cases, an informed consent document must be submitted with an IRB Request for Review. Please use this form and modify as needed. For online surveys, please submit the text that will be displayed on the online survey to serve as an informed consent.

IRB Request for Protocol Renewal
Request a renewal of a previously approved IRB study. A current Informed Consent document must also be submitted.

The IPF is required of all submissions. This form is used for internal approvals and tracking of all submissions.

Grants and Contracts Management

ARRA/Recovery Act Principal Investigator (PI) Reporting Form
Any PI receiving a grant or contract funded by the Recovery Act must complete this internal report by the fifth of the month for activities from the previous month. Data reported will be aggregated and reported to the Federal Government on a quarterly basis.
